As we grow our practice, it is very important for us to
hear your opinions.
Your opinions will help us to streamline our procedures, and
patient interactions.
You are invited to participate in our Patient response survey.
In this survey, approximately 200 people will be asked to
complete questions about their experience. It will take approximately
three (3) minutes to complete the questionnaire.
Your participation in this study is completely voluntary.
There are no foreseeable risks associated with this project.
However, if you feel uncomfortable answering any questions,
you may skip the question. Your survey responses will be strictly
confidential and data from this research will be reported
only in the aggregate (results posted 3/31/07 at on the website).
